We are so excited to welcome you into our dental family and wanted to share a little info ahead of time so that you know what to expect during your New Patient Visit with us.
You should receive a text message or email from us, depending on your preferences, containing a link to complete your new patient paperwork. Completing the paperwork before your appointment will ensure that we can devote the entirety of your new patient visit to treating you. If you cannot complete the paperwork at home, please let us know, and we will be happy to supply you with an iPad to complete the paperwork in the office twenty minutes before your scheduled visit.

We ask that patients...
|Cooperate in making and keeping appointments.
|Cancellation Policy: Appointment changes need to be made 48 hours (excluding weekends) in advance to allow for another patient to be seen and to avoid a cancellation fee.
|Make payment of fees at time of treatment unless arrangements have been made in advance. (Check, Cash, VISA, M/C, AMEX, Discover accepted as well as financing available through Wells Fargo Bang or CareCredit)
As a courtesy, we are happy to provide a complimentary benefits check for those who have dental insurance, and for those who do not, we do offer our own Membership Savings Plan.
